Pada umumnya terjadi selama masa bayi dan anak-anak dan sering berhubungan dengan peningkatan kadar IgE dalam serum serta riwayat atopi pada keluarga atau penderita. Sinonim dari penyakit ini adalah eczema atopik, eczema konstitusional, eczema fleksural, neurodermatitis diseminata, prurigo Besnier. Jari tangan teraba dingin. d. Terdapat papul, likenifikasi, eritema, erosi, eksoriasi, eksudasi dan krusta pada lokasi predileksi. Predileksi Dermatitis Atopik a. Tipe bayi infantil Dahi, pipi, kulit kepala, leher, pergelangan tangan dan tungkai, serta lutut pada anak yang mulai merangkak Lesi berupa eritema, papul vesikel halus, eksudatif, krusta. b. Tipe anak Lipat siku, lipat lutut, pergelangan tangan bagian dalam, kelopak mata, leher, kadang-kadang di wajah. Lesi berupa papul, sedikit eksudatif, sedikit skuama, likenifikasi, erosi. Kadang-kadang disertai pustul. c. Tipe remaja dan dewasa Lipat siku, lipat lutut, samping leher, dahi, sekitar mata, tangan dan pergelangan tangan, kadang-kadang ditemukan setempat misalnya bibir mulut, bibir kelamin puting susu, atau kulit kepala. Lesi berupa plak papular eritematosa, skuama, likenifikasi, kadangkadang erosi dan eksudasi, terjadi hiperpigmentasi. Berdasarkan derajat keparahan terbagi menjadi Dermatitis Atopik sedang apabila mengenai kurang dari 10-50% luas permukaan kulit. Dermatitis Atopik berat apabila mengenai kurang dari > 50% luas permukaan kulit. Tanpa penyulit umumnya tidak diikuti oleh infeksi sekunder. Dengan penyulit disertai infeksi sekunder atau meluas dan menjadi relekalsitran tidak membaik dengan pengobatan standar. Pemeriksaan Penunjang Pemeriksaan IgE serum Diagnosis Klinis Diagnosis Dermatitis Atopik ditegakkan berdasarkan anamnesis dan Pemeriksaan Fisik harus terdiri dari 3 kriteria mayor dan 3 kriteria minor dari kriteria Williams 1994 di bawah ini. Muka pucat atau eritem. Gatal bila berkeringat. Intolerans terhadap wol atau pelarut lemak. Aksentuasi perifolikular. Hipersensitif terhadap makanan. Perjalanan penyakit dipengaruhi oleh factor lingkungan dan atau emosi. Tes kulit alergi tipe dadakan positif. Kadar IgE dalam serum meningkat. Mulai muncul pada usia dini. Diagnosis Banding Dermatitis Atopik Dermatitis seboroik terutama pada bayi, Dermatitis kontak, Dermatitis numularis, Skabies, Iktiosis, Psoriasis terutama di daerah palmoplantar, Sindrom Sezary, Dermatitis herpetiformis. Pada bayi, Diagnosis banding, yaitu Sindrom imunodefisiensi misalnya sindrom Wiskott-Aldrich, Sindrom hiper IgE. Komplikasi Infeksi sekunder Perluasan penyakit eritroderma Penatalaksanaan dan Pengobatan Dermatitis Atopik a. Penatalaksanaan dilakukan dengan modifikasi gaya hidup, yaitu Menemukan faktor risiko Menghindari bahan-bahan yang bersifat iritan termasuk pakaian sepert wol atau bahan sintetik Memakai sabun dengan pH netral dan mengandung pelembab Menjaga kebersihan bahan pakaian Menghindari pemakaian bahan kimia tambahan Membilas badan segera setelah selesai berenang untuk menghindari kontak klorin yang terlalu lama Menghindari stress psikis Menghindari bahan pakaian terlalu tebal, ketat, kotor Pada bayi, menjaga kebersihan di daerah popok, iritasi oleh kencing atau feses, dan hindari pemakaian bahan-bahan medicatedbaby oil Menghindari pembersih yang mengandung antibakteri karena menginduksi resistensi b. Untuk mengatasi keluhan, farmakoterapi diberikan dengan Topikal 2x sehari Pada lesi di kulit kepala, diberikan kortikosteroid topikal, seperti Desonid krim catatan bila tidak tersedia dapat digunakan fluosinolon asetonidkrim selama maksimal 2 minggu. Pada kasus dengan manifestasi klinis likenifikasi dan hiperpigmentasi, dapat diberikan golongan betametason valerat krim atau mometason furoat krim Pada kasus infeksi sekunder, perlu dipertimbangkan pemberian antibiotik topikal atau sistemik bila lesi meluas. Oral sistemik Antihistamin sedatif yaitu hidroksisin 2 x 1 tablet selama maksimal 2 minggu, atau Loratadine 1×10 mg/ hari atau antihistamin non sedatif lainnya selama maksimal 2 minggu Pemeriksaan Penunjang Lanjutan Pemeriksaan untuk menegakkan atopi, misalnya skin prick test/tes uji tusuk pada kasus dewasa. Konseling dan Edukasi Penyakit bersifat kronis dan berulang sehingga perlu diberi pengertian kepada seluruh anggota keluarga untuk menghindari faktor risiko dan melakukan perawatan kulit secara benar. Memberikan informasi kepada keluarga bahwa prinsip pengobatan adalah menghindari gatal, menekan proses peradangan, dan menjaga hidrasi kulit. Menekankan kepada seluruh anggota keluarga bahwa modifikasi gaya hidup tidak hanya berlaku pada pasien, juga harus menjadi kebiasaan keluarga secara keseluruhan. Rencana Tindak Lanjut Diperlukan pengobatan pemeliharaan setelah fase akut teratasi. Pengobatan pemeliharaan dengan kortikosteroid topikal jangka panjang 1 kali sehari dan penggunaan krim pelembab 2 x sehari, sepanjang waktu. Pengobatan pemeliharaan dapat diberikan selama maksimal 4 minggu. Pemantauan efek samping kortikosteroid. Bila terdapat efek samping, kortikosteroid dihentikan. Prognosis Prognosis pada pasien Dermatitis Atopik umumnya bonam, dapat terkendali dengan pengobatan pemeliharaan. Dilakukan skrining pada keluarga dan perbaikan lingkungan keluarga untuk menjaga tetap bersih dan kering. b. Pasien kontrol kembali apabila dalam 3 hari tidak ada perbaikan dengan obat anti jamur.
